Epson LQ2190- paper torn at the upper and corner each time printing

Hi there..
My company have one printer Epson LQ2190 and was purchased on 2012. Lately this printer starts giving problem. I never face this kind of weird problem. The paper torn at the upper and corner each time user print something. You may look at the picture attached. I am looking for any opinion from you guys or maybe someone that might face the same issue to share your experience with me :)
