Naming custom paper sizes - iP4500

In the past I have set up custom paper sizes in the iP4500 printer dialog and given them names. I must have forgotten how, because when I fill in the size under the "Custom" option there seems nowhere to give it a name. Nor can I, it seems, delete the ones I've already set. I've got "advanced options" checked in the printer setup.

What have I forgotten?
