Xerox Phaser 6280 fuser/exit feed issue

First issue: Our printer is randomly "catching" paper on the top middle edge as it exits the top part of the machine; it's almost as though a piece has broken off (where the silver part is showing in the photo) and it's catching the paper there, causing it to jam or become an accordion on its way out of the machine. Weird thing is this doesn't happen all the time. Second issue: when we go to print something, the printer sometimes beeps intermittently until we walk over and press the OK button a few times or reinsert the paper in the tray and only then will it print. Any advice would be appreciated. It's a great machine other than these two recent quirks.
